CONDITIONS · HIS FERTILITY · LOW SPERM COUNT

One low report is not a diagnosis.

A low sperm count (oligospermia) means fewer sperm in the semen than the WHO reference limit. It is not the same as having no sperm, and it often has a cause that can be treated.

  • Common
  • Repeat the test first
  • Often a treatable cause
  • ICSI needs very few sperm

01

What does oligospermia mean?

Oligospermia means a sperm count below the WHO 2021 lower reference limit: fewer than 16 million sperm per ml, or fewer than 39 million in the whole sample.

Those limits are the level of the lowest 5% of men whose partners conceived within a year. They are not a pass mark. Men below them father children naturally, and men above them sometimes cannot. Doctors often call it mild, moderate or severe; severe means only a few million per ml.

02

Can you get pregnant with a low sperm count?

Yes, many couples conceive with a mildly low count, but the lower the count, the longer it tends to take and the more likely treatment is needed.

Count is one number of several. Movement, shape and volume are read with it. Her age, egg reserve and tubes matter just as much. That is why the plan is made for both of you, not from his report alone.

03

Why test again, and what else is checked?

A low count must be confirmed on a second sample, a few weeks to three months later, because counts vary a lot in the same man.

A fever, a recent illness, a short gap since last ejaculation, or a spilled sample can all read falsely low. Give the sample after 2 to 7 days without ejaculation, collect all of it, use no lubricant, and get it to the lab within an hour. If it is still low: an examination for varicocele and testis size, and hormone tests (FSH, LH, testosterone, prolactin, thyroid). When the count is very low, usually under 5 million per ml, guidelines advise a chromosome test and a Y-chromosome test.

04

What causes a low sperm count, and how is it treated?

Common causes are varicocele, testosterone or steroid use, hormone problems, genetic causes, past infection, heat, smoking, alcohol, excess weight and some medicines, and treatment starts with whichever of these is found.

Stopping steroids, correcting a hormone deficiency, treating infection or thyroid disease, controlling diabetes, and repairing a significant varicocele in selected men can all raise the count. Hormone tablets without a proven deficiency are not a treatment. In many men no cause is found; that does not stop treatment.

Honest note

Even when ICSI is planned, a correctable cause is still worth treating. ICSI solves fertilisation, not sperm quality, and a change made 2 to 3 months before a cycle can improve the sample on the day.

Why treat low sperm count before ICSI
05

IUI or ICSI: which one for a low sperm count?

IUI can suit a mild low count when enough moving sperm are left after washing and her tests are normal; IVF with ICSI is the better route when the count is low or other factors are present.

ICSI injects one sperm into each egg, so it needs only a few. IUI is simpler but needs more moving sperm and at least one open tube. If IUI is tried, agree the number of cycles first, so it does not drift on for a year.

Honest note

ICSI should not simply be viewed as automatically better IVF. (Dr Vani Sundarapandian, Medical Director)

06

What can he do, and what won't lifestyle fix?

Stopping smoking, cutting alcohol, losing excess weight, keeping the testes cool and stopping testosterone or steroids are worth doing, and in a mild case they can raise the count.

Changes take at least 3 months to show in a report. They will not correct a genetic cause or reverse severe testicular damage. Expensive antioxidant courses have weak evidence for improving the chance of a baby.

Does this sound like you?

If any of these fit, a repeat test and a consultation is the next step.

  • My report says oligospermia, or low count
  • I've had only one semen test
  • I use testosterone or gym supplements
  • I drive long hours or work in heat
  • We've tried for a year with no pregnancy
  • A doctor has suggested ICSI
Yes, recheck my count

Report says no sperm at all? That is a different question.

What we try, in order.

  1. 1

    Repeat the semen analysis

Correct collection, a few weeks to 3 months after the first.

  2. 2

    Find a cause

Examination, hormone tests, and genetic tests when the count is very low.

  3. 3

Treat the cause and change what he can

Hormones, infection, steroids, varicocele, weight, smoking, heat. Recheck after about 3 months.

  4. 4

    IUI, if mild

When enough moving sperm remain after washing, with a cycle limit agreed first.

  5. 5

    IVF with ICSI

When the count is low or IUI has had its fair chance.

Questions men ask

What is a normal sperm count?

Under WHO 2021, a sperm count of 16 million per ml or more, and 39 million or more in the whole sample, is within the reference range. It is a reference point, not a promise either way.

Can low sperm count be cured?

A low sperm count can often be improved when a cause is found and treated, such as steroid use, a hormone problem, infection or a varicocele. When no cause is found, IUI or ICSI works around it.

How long does it take to improve sperm count?

Sperm takes about 2 to 3 months to make, so any change shows in a report after about 3 months. Testing sooner can miss it.

Where can I get a sperm count test in Chennai?

Jananam Fertility Centre does semen analysis at Neelankarai, off ECR, and at Thiruporur. Message us on WhatsApp for a time and the collection instructions.

Do I need ICSI for a low sperm count?

Not always: a mildly low count can suit timed intercourse or IUI if her tests are normal. ICSI is advised when the count is low, movement is poor, or IUI has not worked.

Do supplements increase sperm count?

The evidence that antioxidant supplements improve the chance of a baby is weak. Stopping smoking, cutting alcohol and reaching a healthy weight have clearer effects.
