especially want to mention Aishwarya, the nurse, who was amazing and went above and beyond her duties. Dr. Vani treated us like family, making each patient feel valued.

CONDITIONS · SECONDARY INFERTILITY
The first pregnancy came easily. The second doesn't have to be a mystery.
Secondary infertility is difficulty getting pregnant after you have been pregnant before. A first pregnancy shows it was possible then; it does not mean nothing has changed since.
- Common, and often has a cause
- Both partners tested again
- Same timelines as a first baby
- Caesarean scar checked
- Ovaries — Egg reserve falls with age
- Womb — Caesarean scar niche or scarring inside
- Tubes — Infection or surgery after delivery
- Sperm — Count and movement can change
01What is secondary infertility?
Secondary infertility is difficulty getting pregnant after at least one previous pregnancy, whether or not that pregnancy ended in a birth.
It is more common than most couples expect. Family and friends often say 'you did it before, just relax', and couples wait. A previous pregnancy is reassuring, but it is not a reason to wait longer than anyone else would.
02Why is it harder to get pregnant the second time?
It is often harder the second time because things have changed since the first pregnancy: age, egg reserve, the womb, the tubes, sperm, or general health.
Egg reserve falls with age, and a few years makes a difference after 35. Weight, thyroid and blood sugar may have shifted. Endometriosis, fibroids or PCOS can progress. His sperm count and movement can fall too. Often there is a clinical reason, and nobody has looked for it.
03Can a caesarean or a difficult delivery cause secondary infertility?
Sometimes, yes: a caesarean can leave a small pouch in the scar (a niche), and infection or surgery after a delivery can scar the tubes or the inside of the womb.
A niche can collect blood and fluid, which may show as spotting for days after a period. A D&C for retained placenta can leave scarring inside the womb, sometimes with lighter periods. An infection after delivery can damage the tubes silently. That is why we image the womb and tubes rather than assume they are unchanged.
Caesarean scar niches are common, but direct evidence that they reduce fertility is still limited, and repair has not yet been proven to improve pregnancy in trials. We check for a niche; we do not operate on one by reflex.
04What tests are done for secondary infertility?
The tests for secondary infertility are the same as for a first pregnancy: semen analysis, egg reserve, a pelvic scan, a tube test and hormone checks.
The scan looks at the caesarean scar as well as the womb and ovaries. If the history points to scarring inside the womb or a niche, a hysteroscopy may follow. Treatment then depends on what is found: ovulation tablets, IUI, IVF, or treating the womb first.
Semen analysis: what it measures05When should we see a doctor about a second baby?
See a doctor after 12 months of trying if the woman is under 35, after 6 months from 35, and straight away from 40.
Go sooner if you had a complicated delivery, a caesarean with spotting after periods, an infection after birth, a D&C, or irregular periods since. Breastfeeding can delay periods returning; if periods have not settled after you stop, get checked.
06How do we cope with secondary infertility?
Secondary infertility is a real loss, and it is fine to grieve it even when you already have a child.
Many parents feel guilty for wanting more, or unseen because 'at least you have one'. Talking it through with your partner and with us helps. Counselling is available at Jananam if you want it. You can care for the child you have and still want another.
Does this sound like you?
If any of these sound like you, it is worth testing now rather than waiting.
- We have one child and have tried for a year or more
- I'm over 35 and trying for our second
- I had a caesarean and now spot for days after my period
- My delivery was complicated or I had an infection after
- Everyone says relax, it happened before
- His tests were never repeated

What we do, in order.
- 1
Test both of you again
Semen analysis, AMH, scan with the caesarean scar checked, tube test, thyroid and sugar.
- 2
Treat what we find
Thyroid, weight or blood sugar corrected. Problems inside the womb dealt with if they matter.
- 3
Tablets or IUI
If tubes are open and sperm are good enough, with a cycle limit agreed by age.
- 4
IVF
Blocked tubes, low reserve, low sperm, or age that makes time the priority.

Questions patients ask
What is secondary infertility?
Secondary infertility is difficulty getting pregnant after a previous pregnancy. It is common and often has a treatable cause.
Why can't I get pregnant with my second baby?
The most common reasons are age and falling egg reserve, changes in his sperm, weight or thyroid changes, and effects of the first delivery on the womb or tubes. Testing both partners usually shows which one applies.
Can a C-section cause secondary infertility?
A caesarean can occasionally leave a scar niche or adhesions that may affect fertility, though the evidence on niches is still limited. A scan checks for it.
Does male fertility change after the first child?
Yes, sperm count and movement can fall with age, weight gain, smoking, illness or some medicines. His semen analysis should be repeated, not assumed normal.
Can breastfeeding cause secondary infertility?
Breastfeeding can delay ovulation and periods while you are feeding often. Once you stop and periods return, fertility usually returns; if periods stay irregular, get checked.
Is secondary infertility treatable?
Yes, secondary infertility is treated the same way as a first pregnancy problem: treat the cause, then tablets, IUI or IVF as needed. The earlier you test, the more options you have.
