TESTS · HSG, HYCOSY AND HYFOSY
HSG, HyCoSy and HyFoSy pass a fluid through the womb and watch whether it flows out of both tubes. They answer one question: can an egg and sperm meet?
An HSG test is an X-ray that shows whether your fallopian tubes are open and whether the inside of your womb has a normal shape.
The egg and sperm meet inside the tube, and the embryo travels down it to the womb. If both tubes are blocked, tracking ovulation or IUI cannot work, and IVF is the route because it bypasses the tubes. If one tube is open, natural pregnancy and IUI are still possible. That difference is why the test is done rather than guessed.
Fallopian tube problemsA thin tube is passed through the cervix, a dye goes in slowly over a minute or two, and X-ray pictures are taken; most women feel cramping like a strong period for a few minutes.
You lie with your knees bent, a speculum is placed as for a smear test, and the cervix is cleaned. There is no anaesthetic and no cut. A few women find it sharply painful: say so straight away, because slowing the dye usually helps. Light spotting and a sticky discharge for a day or two are normal. Fever, heavy bleeding or pain that gets worse after 24 hours needs a same-day check: call us on +91 89399 94244; outside clinic hours, call 108 or go to the nearest emergency department, then call us.
An HSG can hurt. Taking a simple painkiller such as ibuprofen about an hour before is recommended, and we tell you exactly what to take before the day. (ASRM patient fact sheet)
The test is done after your period stops and before you ovulate, usually between day 6 and 11 of the cycle.
Bleeding would blur the pictures, and testing before ovulation means there is no chance of an early pregnancy being present. The appointment is booked by counting from day 1 of your period. If your period comes early or late, tell us and we move it. It is also postponed if you might be pregnant, have a pelvic infection or are bleeding heavily.
A tube test shows whether the tubes are open, not whether they work well inside, and it does not show endometriosis, adhesions outside the tubes or the ovaries.
The fine hairs that move the egg along cannot be seen on X-ray or ultrasound. A tube can also look blocked when it is only in spasm at the point where it joins the womb. One unexpected result is a reason to repeat or cross-check, not to conclude.
All three ask the same question; they differ in how they see: X-ray (HSG), ultrasound (HyCoSy with saline, HyFoSy with foam gel), or a camera under anaesthetic (laparoscopy).
NICE advises HSG as the first test for women with no history of pelvic infection, surgery or endometriosis, with HyCoSy an alternative where the expertise exists. HyCoSy and HyFoSy avoid X-ray and iodine and show the ovaries at the same time. Laparoscopy is surgery, so it is kept for suspected endometriosis, past pelvic infection or surgery, or an unclear first test, and it can treat what it finds.
Bring your HSG or HyCoSy report and images. We read them with you, line by line.

The inside of the womb looks a normal shape, with nothing growing into it.
Something is taking up space inside the womb: often a polyp, a fibroid or scar tissue.
The dye or foam travels along the tube.
Fluid leaves the far end of both tubes, so both tubes are open (patent).
One tube did not let fluid through during the test.
The block is where the tube joins the womb.
The far end of the tube is closed and the tube is swollen with fluid.
Fluid comes out but pools around the tube instead of spreading, which suggests scar tissue outside it.
The womb has an unusual shape, usually present from birth.
Source of ranges: There are no numbers on this report. The normal finding (normal cavity, both tubes fill, free spill both sides) follows the ASRM HSG fact sheet and the ASRM fertility evaluation committee opinion (2021); test choice follows NICE NG257.
IfBoth tubes open, normal cavity
The tubes are ruled out. We move on to ovulation, sperm and egg reserve, and timed intercourse or IUI stay on the table.
What that meansIfOne tube open
Natural pregnancy and IUI are still possible. The open side is taken into account when timing.
What that meansIfBlocked at the womb end, possibly spasm
We repeat or cross-check with another test before deciding anything.
What that meansIfBoth tubes blocked
IVF is the treatment that works, because it bypasses the tubes. Better known now than after months of treatment that cannot help.
What that meansIfHydrosalpinx (swollen tube)
Usually treated before an embryo transfer, because the fluid lowers the chance of implantation.
What that meansIfPolyp, fibroid or scarring in the cavity
Confirmed and usually removed at hysteroscopy, often in the same short procedure.
What that meansA tube check is usually part of the first round of tests if any of these is you.
How long you've tried, past infections, surgery or ectopic pregnancy, and any tube test already done.
The tube test that fits your history, booked for day 6 to 11, with his semen analysis.
What the result means for you: try naturally, IUI, IVF, or treat a hydrosalpinx first.
The treatment the tubes allow, started without waiting months on the wrong one.
A normal HSG report says the womb cavity is normal in size and outline, both tubes fill along their length, and dye spills freely from both ends. Some reports simply write 'bilateral free spill' or 'no filling defect'.
Most women feel period-like cramping for a few minutes during an HSG, not sharp pain. A painkiller taken about an hour before helps, and slowing the dye helps if it hurts more.
HyCoSy and HyFoSy are the same ultrasound tube test; HyCoSy uses saline or a contrast fluid, and HyFoSy uses a foam gel that is easier to see on the scan. Neither uses X-ray or iodine.
Neither is better for everyone, because both check whether the tubes are open. HSG uses X-ray and gives a lasting image; HyCoSy uses ultrasound with no radiation and shows the ovaries too. We choose with you based on your history.
An HSG does not reliably unblock a damaged tube, but some couples conceive in the months after it, possibly because the fluid flushes the tubes. It is a side benefit, not a reason to have the test.
Yes, you can try in the same cycle once the test is done, because it is done before ovulation. Many doctors encourage it.
With one open tube, natural pregnancy and IUI are still possible. A single blocked tube is usually watched rather than treated, unless it is swollen with fluid.
Yes, endometriosis is a reason to check the tubes, not a reason to avoid it. It may change which test is chosen, often towards laparoscopy, which can treat the endometriosis at the same time.
